fix: resumable Actions ignore project script overrides #219

Project Actions added or edited through the desktop UI are absent from the resumable MCP Action list. This prevents an agent from launching a newly saved, explicitly opted-in Action.

The chat UI persists changes through projectScriptOverrides settings in apps/web/src/components/ChatView.tsx. ActionResume.listProjectActionsImpl and its launch lookup instead read project.scripts from the orchestration snapshot in apps/server/src/actionResume/ActionResume.ts.

Reproduction: add a uniquely named Action through Script actions, enable Allow Codex and Claude to run and resume, save, and call list_project_actions. The UI menu shows the new Action while MCP returns only the old project script list. A read-only check of the project projection confirms it contains the old list.

Expected: list and launch resolve the same effective scripts and opt-in flags as the UI, preserving the current continuation guard. No runtime restart or database editing should be required.

Observed while delivering the migration-history fix. Workaround for that delivery is an existing opted-in Action invoking a temporary self-restoring launcher that delegates to the intended clean worktree.
