correct ancient URL errors. update README for 2019

really fix tests

bring tests into conformity with instructions.

(reflection was looking for ${githubid}.md, which is no longer the
right place for that file.)

final readme improvements

minor improvements based on previous assignments issues

mostly testing, git ocnflicts, etc.

adds accessibility to index.html

remove security vulnerabilities & extraneous phantom packages

assignmnt is largely complete, and tests work.
