Capitalization and Minor Improvements

* ubuntu should be Ubuntu
* Ubuntu 12.04 should be Ubuntu Server 12.04 or Ubuntu Server 12.04 LTS (preferred by Ubuntu Community)
* Consistent capitalization of "Discourse"
