Full source review of `issue/216-B`

@ingydotnet I fixed one of my main concerns for the new upcoming 0.4.0 last friday. There was a problem when you recreated the structure using --first-parent.

@ingydotnet There are two issue that we need to decide:

1. -M/-R flag for pull operation. Currently you select a method with clone/init, you can later change it with git subrepo config.
2. Default mode? Currently it's set to merge because it will avoid extra conflicts.
