Handle Corilus Medidoc DMA-LAB format

• Create a "versioned" corilus-medidoc DB table that can be automagically updated by dropping zip files from https://labcodes.corilus.be/ (extract version and kind, keyword or unit, from zip file name).
• Create a custom lab report table for DMA-REP files, possibly by refactoring the existing report table code for Healthone.
