Add a "Other" textbox on the observation screen

Add a non-mandatory textbox that is permanently visible on the observation screen. This will allow for additional comments to be typed in to describe the "Other" choice. As in, what other behaviours were visible under this tickbox?

The contents from this textbox can then be printed on the final results page (optional, with a tickbox). "Include "Other" text" yes or no.
