Guidance hints

Add support for the guidance form for labels. This is used to provide additional guidance about a question that isn't always needed and so is collapsed by default.

Make sure <outputs> are supported inside the guidance.

I updated the issue to link to the spec! It's not an essential or very common feature so while it's necessary to support the full form spec, I think it's a good example of something we could add shortly after a 1.0 release. At the same time, it's probably a quick win so we could also prioritize on that basis.

I was surprised to see that the guidance text form actually is displayed already:

I assume any unknown text form would be treated this way for now?
