What

Addresses agentflare item 595 (a tracker item, not a GitHub issue): a merged PR whose footer named the wrong item silently auto-completed that item with zero implementation.

Incident

Another item was auto-completed because find_duplicate_pr matched a merged PR whose footer named that item but whose content was a different item's work. The visible item-number footer is only unique per project per workstation (each keeps its own item DB), so it can attribute one item's PR to another that shares the number.

Changes

  • pr_footer now also appends a hidden HTML-comment tag carrying the item UUID. marks_this_item / find_by_item_marker reject a PR tagged for a different item. Untagged legacy PRs still fall back to the number.
  • handle_duplicate_pr: a merged match now skips dispatch and flags the item for human review, exactly like an open match, instead of mark_completed + cleanup_worktree + relabel_pr_completed on a text marker alone.
  • Regression tests for the incident shape (two items sharing one number) and for a merged duplicate never completing an item.
  • Moved the inline tests of src/worktree/mod.rs to src/worktree/mod_tests.rs (include!, the same way cli/work.rs does) so the file clears the 1500-line LOC gate. That is most of the diff: about 700 lines moved, not new logic.

Scope, please read

This closes Bug 2 (merged duplicate auto-completes with no human gate) and hardens detection for Bug 1, but it does NOT root-cause how a worktree/branch created for one item ended up named and stamped with another item's number. The item's "done when" asks for that root cause plus a regression test on the naming path. Suggest keeping item 595 open for that part rather than closing it on merge.

Notes for review

  • Please read the src/github/pulls.rs change (96 lines) closely; it was not in the original plan's file list.
  • Not run locally by the person opening this PR; CI is the check.

Opened by claude-code for item #595 (pushed from a PM session because the agent's own job was orphaned by a daemon restart).

Summary by CodeRabbit

  • Bug Fixes

    • Improved duplicate pull request detection by distinguishing items with unique identifiers.
    • Prevented unrelated pull requests from being incorrectly associated with an item.
    • Stale, CI-failing duplicates no longer block processing.
    • Merged or open duplicates are no longer automatically completed or modified; instead, processing pauses for human review and the claim is released.
  • Reliability

    • Improved handling of pull request status checks, metadata, branch updates, and recovery from concurrent pull request creation.

📝 Walkthrough

Walkthrough

The change adds item ID tags to PR matching and propagates them through worktree operations. Duplicate PR handling now releases claims and requests human review without automatically completing items or cleaning worktrees.

Changes

PR identity and duplicate handling

Layer / File(s) Summary
Item identity matching
src/github/pulls.rs
PR matching now validates both sequence markers and item ID tags. Conflicting tags are rejected, while untagged legacy PRs remain valid.
Worktree PR identity propagation
src/worktree/mod.rs, src/worktree/mod_tests.rs
Worktree PR creation and ownership checks now use item IDs. The extracted tests cover PR metadata, CI status, ownership, persistence, and recovery behavior.
Duplicate PR handling
src/cli/work_duplicate_pr.rs, src/cli/work_duplicate_pr_tests.rs
Duplicate handling now passes item IDs, releases claims, skips dispatch, and requests human review. Merged duplicates no longer complete items, clean worktrees, or relabel PRs.

Do not report success when the human-review comment fails.

handle_duplicate_pr ignores comment_impl errors, releases the claim, and returns a successful WorkOutcome. A failed comment can therefore leave no persistent human-review signal. Return a failed outcome so the caller can retry or surface the failure.

Require item identity for open branch matches.

is_own_pr accepts every open PR before calling marks_this_item. An open PR tagged for another item can therefore be accepted by push_and_open_pr, and its number can be persisted as this item's authoritative PR identity. Require marks_this_item for open matches too.

Replace is_own_pr_true_for_an_open_pr_regardless_of_body with matching and conflicting-tag cases. Do not add body validation to the metadata-first merge and CI paths. Those paths intentionally trust the persisted PR number, including when the fetched body is absent or differs.

Reject conflicting or malformed identity tags.

tag_allows checks only the first ITEM_ID_TAG_PREFIX. A fetched PR body can contain a matching tag followed by a conflicting or malformed tag. The first tag then makes marks_this_item accept an ambiguous identity.

Inspect every identity-tag prefix. Reject malformed tags and any tag whose value differs from item_id.

…carry item UUID (#595)

Item #198 was auto-completed with zero implementation because
find_duplicate_pr matched PR #636, whose footer said "for item #198" but
whose content was a different item's work. The visible `for item #<N>`
marker is only unique per project per workstation (each keeps its own
item DB), so it can attribute one item's PR to another that shares the
number.

- pr_footer now appends a hidden `<!-- agentflare-item-id: <uuid> -->`
  tag; marks_this_item / find_by_item_marker reject a PR tagged for a
different item. Untagged legacy PRs still fall back to the number.
- handle_duplicate_pr: a merged match now skips dispatch and flags for
human review like an open match, instead of mark_completed +
  cleanup_worktree + relabel_pr_completed on a text marker alone.
- Regression tests for the incident shape (two items sharing #198) and
  the merged-duplicate-never-completes behavior.
- Move worktree/mod.rs inline tests to mod_tests.rs (include!, as
cli/work.rs does) so the file clears the 1500-line LOC gate.
