No Approval Request, silent fail

I am trying out ralph with the "ralph --monitor" command.
For some reason it does not block execution when it tries to run a command like "npm install" to ask for permission but rather just logs that it did not get permission and keeps looping.
Im on wsl2 for windows if that matters.

Same issue, would be nice to have a way to pass params to the claude command.

This appears to be a bug with permission handling. To help diagnose:

1. What version of Ralph are you running? (ralph --version or check install date)
2. What version of Claude Code CLI? (npx @anthropic/claude-code --version)
3. Are you running Ralph directly or via ralph --monitor with tmux?
4. Can you share the relevant section from .ralph/logs/ralph.log when the silent failure occurs?

The issue may be related to how tmux handles stdin/stdout or how WSL2 processes interactive prompts.
