When an ActiveEffect is created on an item, the icon and label/name should default to the item's img and name #2444

This seems like an appropriate default to have for ActiveEffects and speeds up item and effect creation.

  • For example, the bless spell applies 'Bless' and the Rage feature applies 'Rage'.
  • Identical images allow for a mental link at a glance.
