When an ActiveEffect is created on an item, the icon and label/name should default to the item's img and name #2444

This seems like an appropriate default to have for ActiveEffects and speeds up item and effect creation.

For example, the bless spell applies 'Bless' and the Rage feature applies 'Rage'.
Identical images allow for a mental link at a glance.
