Slice: Replace transient why toast with anchored evidence panel #240

Goal

Render :why in a readable anchored panel near the requested line instead of a short-lived toast.

Acceptance criteria

• Panel appears below the line when space exists and above otherwise.
• Panel remains until cursor moves away, Escape, or explicit close.
• Panel uses the same evidence model as machine-readable :why output.
