Random errors on order placement

I have been observing quite a few of these recently:

in response to the message order placement message. It seems random, erratic.

InitPostgres would imply it's an issue with his server writing info to database? Possible locking issue when under load?

A different one this time:

A lot of errors in general in the platform today.
