Milestone: post-beta-compatibility

Measure request-to-first-byte RTT per source and combine it with bandwidth in a bounded, hysteretic BDP request-depth controller. Preserve the three-range packet constraint, conservative startup/fallback, and prompt reduction on slowdown or timeout.
