[Uptime] ML Monitor alerts not editable / creatable through mgmt

The new ML alerting feature from Uptime only works when clicked from within the UI. It's not editable after the fact, or createable through management.

I propose we expose the monitor ID in the UI so it says WHEN MONITOR 'myMonitorId' where myMonitorId is a text box.

@andrewvc i think we should enable require app context true for this alert, it doesn't make sense allowing creating this alert from alert management app. I mean we need to determine if anomaly detectection is enabled for the monitor to enable alert on the monitor.
