Homogenize ampd configuration: env vars and config

Goal: Make the ampd binary
  configuration consistent across environment variables and config files.

  Scope:
  - Inventory current env vars and config options used by ampd.
  - Normalize naming, defaults, and precedence rules.
  - Update docs and examples to match the new scheme.
  - Ensure any breaking changes are called out.
