What problem does this solve?

Sometimes report generation fails because the model context is full or a network error. The findings are in memory but only findings from the current operation are reported.

Proposed solution

Read findings and related info from memory and report on everything for the target. If memory isolation is set to 'operation', only report on the current operation.
