HttpResponseStreamWriter never calls Flush on the Stream

Not when the user calls it; and not when its Disposed

The Stream is never Flushed; if its a buffered Stream this will cause issues.

It's hard to know when to flush. This doesn't affect most of our streams so closing until somebody else hits this.
