Improve markdownlint configuration

Summary

#9353 added markdownlint to the lint commands in Makefile with a focus on providing warnings and making fixes related to whitespace (see #8112). The configuration for markdownlint was originally copied from tendermint (see markdownlint.yml) and then modified to include additional rules to help limit the scope of #9353.

It would be nice to align our configuration for markdownlint with the configuration in tendermint and to fix the lint warnings.

Following up on #9592, we should also consider including the changelog.

2. remove the CHANGELOG.md from the ignore list
3. run make lint-fix
4. fix the remaining lint issues manually
