Remove duplicated characters

Some PDF use duplicated characters (at the same place) to produce a "fake bold font".

Due to the optimization of pdf2htmlEX, the two characters may not end up to exactly the same, so the result will be terrible.

Need to remove them, like pdftohtml do.
