[FEATURE] Accept only selected diff

Feature Description

It should be possible to only accept a selected Diff and not the entire file.

Use Case

When the generated Code is partly good, i only want a selected diff in one file to be accepted.

Proposed Solution

Make a difference between the entire file with :w and lets say only current hovered diff with a different keymap

When providing diff, claudecode.nvim open a Vim diff session (see :help diff). The default command to accept/reject diff are do/dp (see :help do).
