* Cache Media column listing instead of querying the schema per attribute

Media::isRealAttribute() ran Schema::hasColumn() on every attribute
access, costing one information_schema query each time. The column
listing is now fetched once per connection and table for the process.
Adds Media::flushColumnCache() for tests and a test asserting attribute
reads hit the schema at most once.
