Added Voice 3 Osc and Env CV Outputs

• The voice 3 register read outs for oscillator and envelope are now available as CV outputs
• All input and output jacks are now labelled

Fixed Filter Cutoff

Writes to Filter LO/HI Reg were wrong and caused filter cut off frequency glitches.
