Create new style chaucer-review.csl from template

- delete bibliography element and associated macros
- no ibid
- books: do not print publisher name
- books: do not print collection title (i.e. series)
- article-journal: do not print issue number
- article-journal: print volume number before page number,
  delimited by colon
- article-journal, chapter: print full page range
  at first reference. A point-locator is preceded by "at".
- update metadata
