Update to workshop request form - question about event open to the public

All three of our workshop forms ask if the workshop is open to the public. We would like to change this question on all three forms as follows, so that we have only two options (removing the "other" option).

Please indicate if this workshop will allow registrants outside of your institution to participate.
Help text: It is not a requirement. Selection of participants is at the discretion of the workshop host. 
Radio button: This workshop is OPEN to participants outside of the institution. 
Radio button: This workshop is CLOSED and will not allow other participants to attend. 

Additionally, the response to this should be included when an event is created from the request form, adding a new field to the event model.
