Move diagrams to use Mermaid where appropriate

Mermaid support is now added to the Workbench, we need to switch diagrams using it from SVG to using mermaid code.

Make sure alt-text is preserved.

Also a point about whether Mermaid will play well with screen-readers and other accessibility tools.
