Opening relative paths within the same MD fails.

Hi Boris,

Although relative paths to images work, relative paths within the same MD fail:
When I click on a relative table-of-content link ("2. chapter2") it opens "https://git.domein.org/project/README.md?at=refs%2Fheads%2Fmaster#2.-chapter2" instead of moving to "https://confluence.domein.org/display/project/test#2.-chapter2".

In Git, I have the following text as markdown to create a table of contents:

[2. chapter2](#2\.-chapter2)

# chapter2

Do you have any ideas to solve this issue with relative paths to the same MD?
