When receiving a deauthentication/disassociation frame from a TDLS
peer, a station should not disconnect the current AP, but only
disable the current TDLS link if it's enabled.

Without this change, a TDLS issue can be reproduced by following the
steps as below:

1. STA-1 and STA-2 are connected to AP, bidirection traffic is running
   between STA-1 and STA-2.
2. Set up TDLS link between STA-1 and STA-2, stay for a while, then
   teardown TDLS link.
3. Repeat step #2 and monitor the connection between STA and AP.

During the test, one STA may send a deauthentication/disassociation
frame to another, after TDLS teardown, with reason code 6/7, which
means: Class 2/3 frame received from nonassociated STA.

On receive this frame, the receiver STA will disconnect the current
AP and then reconnect. It's not a expected behavior, purpose of this
frame should be disabling the TDLS link, not the link with AP.

struct dfl_feature_platform_data (and it's mutex) is used
by both fme and port devices, and when lockdep is enabled it
complains about nesting between these locks.  Tell lockdep about
the difference so it can track each class separately.

ifmsh->csa is an RCU-protected pointer. The writer context
in ieee80211_mesh_finish_csa() is already mutually
exclusive with wdev->sdata.mtx, but the RCU checker did
not know this. Use rcu_dereference_protected() to avoid a
warning.

neighbour after it becomes dead. This prevents the nexthop from ever

When both MPOL_MF_MOVE* and MPOL_MF_STRICT was specified, mbind() should
try best to migrate misplaced pages, if some of the pages could not be
migrated, then return -EIO.

There are three different sub-cases:
 1. vma is not migratable
 2. vma is migratable, but there are unmovable pages
 3. vma is migratable, pages are movable, but migrate_pages() fails

If #1 happens, kernel would just abort immediately, then return -EIO,
after a7f40cf ("mm: mempolicy: make mbind() return -EIO when
MPOL_MF_STRICT is specified").

If #3 happens, kernel would set policy and migrate pages with
best-effort, but won't rollback the migrated pages and reset the policy
back.

Before that commit, they behaves in the same way.  It'd better to keep
their behavior consistent.  But, rolling back the migrated pages and
resetting the policy back sounds not feasible, so just make #1 behave as
same as #3.

Userspace will know that not everything was successfully migrated (via
-EIO), and can take whatever steps it deems necessary - attempt
rollback, determine which exact page(s) are violating the policy, etc.

Make queue_pages_range() return 1 to indicate there are unmovable pages
or vma is not migratable.

The #2 is not handled correctly in the current kernel, the following
patch will fix it.

  mbind(..., MPOL_MF_STRICT|MPOL_MF_MOVE) = 0

The setsockopt() would allocate compound pages (16 pages in this test)
for packet tx ring, then the mmap() would call packet_mmap() to map the
pages into the user address space specified by the mmap() call.

When calling mbind(), it would scan the vma to queue the pages for
migration to the new node.  It would split any huge page since 4.9
doesn't support THP migration, however, the packet tx ring compound
pages are not THP and even not movable.  So, the above bug is triggered.

However, the later kernel is not hit by this issue due to commit
d44d363 ("mm: don't assume anonymous pages have SwapBacked flag"),
which just removes the PageSwapBacked check for a different reason.

But, there is a deeper issue.  According to the semantic of mbind(), it
should return -EIO if MPOL_MF_MOVE or MPOL_MF_MOVE_ALL was specified and
MPOL_MF_STRICT was also specified, but the kernel was unable to move all
existing pages in the range.  The tx ring of the packet socket is
definitely not movable, however, mbind() returns success for this case.

Although the most socket file associates with non-movable pages, but XDP
may have movable pages from gup.  So, it sounds not fine to just check
the underlying file type of vma in vma_migratable().

Change migrate_page_add() to check if the page is movable or not, if it
is unmovable, just return -EIO.  But do not abort pte walk immediately,
since there may be pages off LRU temporarily.  We should migrate other
pages if MPOL_MF_MOVE* is specified.  Set has_unmovable flag if some
paged could not be not moved, then return -EIO for mbind() eventually.

With this change the above test would return -EIO as expected.

This endless clk warnings behaviour is well explained by Andrey Smirnov:

"Allocating DMA after registering I2C adapter can lead to infinite
probing loop, for example, consider the following scenario:

    1. i2c_imx_probe() is called and successfully registers an I2C
       adapter via i2c_add_numbered_adapter()

    2. As a part of i2c_add_numbered_adapter() new I2C slave devices
       are added from DT which results in a call to
       driver_deferred_probe_trigger()

    3. i2c_imx_probe() continues and calls i2c_imx_dma_request() which
       due to lack of proper DMA driver returns -EPROBE_DEFER

    4. i2c_imx_probe() fails, removes I2C adapter and returns
       -EPROBE_DEFER, which places it into deferred probe list

    5. Deferred probe work triggered in #2 above kicks in and calls
       i2c_imx_probe() again thus bringing us to step #1"

So revert commit e1ab9a4 ("i2c: imx: improve the error handling in
i2c_imx_dma_request()") and restore the old behaviour, in order to
avoid regressions on existing setups.

Revert the commit bd293d0. The proper
fix has been made available with commit d0a255e ("loop: set
PF_MEMALLOC_NOIO for the worker thread").

Note that the fix offered by commit bd293d0 doesn't really prevent
the deadlock from occuring - if we look at the stacktrace reported by
Junxiao Bi, we see that it hangs in bit_wait_io and not on the mutex -
i.e. it has already successfully taken the mutex. Changing the mutex
from mutex_lock to mutex_trylock won't help with deadlocks that happen
afterwards.

When a local endpoint is ceases to be in use, such as when the kafs module
is unloaded, the kernel will emit an assertion failure if there are any
outstanding client connections:

and even beyond that, will evince other oopses if there are service
connections still present.

Fix this by:

 (1) Removing the triggering of connection reaping when an rxrpc socket is
     released.  These don't actually clean up the connections anyway - and
     further, the local endpoint may still be in use through another
     socket.

 (2) Mark the local endpoint as dead when we start the process of tearing
     it down.

 (3) When destroying a local endpoint, strip all of its client connections
     from the idle list and discard the ref on each that the list was
     holding.

 (4) When destroying a local endpoint, call the service connection reaper
     directly (rather than through a workqueue) to immediately kill off all
     outstanding service connections.

 (5) Make the service connection reaper reap connections for which the
     local endpoint is marked dead.

Only after destroying the connections can we close the socket lest we get
an oops in a workqueue that's looking at a connection or a peer.

When a cpu requests broadcasting, before starting the tick broadcast
hrtimer, bc_set_next() checks if the timer callback (bc_handler) is active
using hrtimer_try_to_cancel(). But hrtimer_try_to_cancel() does not provide
the required synchronization when the callback is active on other core.

The callback could have already executed tick_handle_oneshot_broadcast()
and could have also returned. But still there is a small time window where
the hrtimer_try_to_cancel() returns -1. In that case bc_set_next() returns
without doing anything, but the next_event of the tick broadcast clock
device is already set to a timeout value.

In the race condition diagram below, CPU #1 is running the timer callback
and CPU #2 is entering idle state and so calls bc_set_next().

In the worst case, the next_event will contain an expiry time, but the
hrtimer will not be started which happens when the racing callback returns
HRTIMER_NORESTART. The hrtimer might never recover if all further requests
from the CPUs to subscribe to tick broadcast have timeout greater than the
next_event of tick broadcast clock device. This leads to cascading of
failures and finally noticed as rcu stall warnings

The comment that hrtimer cannot be armed from within the callback is
wrong. It is fine to start the hrtimer from within the callback. Also it is
safe to start the hrtimer from the enter/exit idle code while the broadcast
handler is active. The enter/exit idle code and the broadcast handler are
synchronized using tick_broadcast_lock. So there is no need for the
existing try to cancel logic. All this can be removed which will eliminate
the race condition as well.

On excessive bit errors for the FCP channel ingress fibre path, the channel
notifies us.  Previously, we only emitted a kernel message and a trace
record.  Since performance can become suboptimal with I/O timeouts due to
bit errors, we now stop using an FCP device by default on channel
notification so multipath on top can timely failover to other paths.  A new
module parameter zfcp.ber_stop can be used to get zfcp old behavior.

User explanation of new kernel message:

 * Description:
 * The FCP channel reported that its bit error threshold has been exceeded.
 * These errors might result from a problem with the physical components
 * of the local fibre link into the FCP channel.
 * The problem might be damage or malfunction of the cable or
 * cable connection between the FCP channel and
 * the adjacent fabric switch port or the point-to-point peer.
 * Find details about the errors in the HBA trace for the FCP device.
 * The zfcp device driver closed down the FCP device
 * to limit the performance impact from possible I/O command timeouts.
 * User action:
 * Check for problems on the local fibre link, ensure that fibre optics are
 * clean and functional, and all cables are properly plugged.
 * After the repair action, you can manually recover the FCP device by
 * writing "0" into its "failed" sysfs attribute.
 * If recovery through sysfs is not possible, set the CHPID of the device
 * offline and back online on the service element.

A long time ago we fixed a similar deadlock in show_slab_objects() [1].
However, it is apparently due to the commits like 01fb58b ("slab:
remove synchronous synchronize_sched() from memcg cache deactivation
path") and 03afc0e ("slab: get_online_mems for
kmem_cache_{create,destroy,shrink}"), this kind of deadlock is back by
just reading files in /sys/kernel/slab which will generate a lockdep
splat below.

Since the "mem_hotplug_lock" here is only to obtain a stable online node
mask while racing with NUMA node hotplug, in the worst case, the results
may me miscalculated while doing NUMA node hotplug, but they shall be
corrected by later reads of the same files.

I think it is important to mention that this doesn't expose the
show_slab_objects to use-after-free.  There is only a single path that
might really race here and that is the slab hotplug notifier callback
__kmem_cache_shrink (via slab_mem_going_offline_callback) but that path
doesn't really destroy kmem_cache_node data structures.

Daniel Vetter uncovered a nasty cycle in using the mmu-notifiers to
invalidate userptr objects which also happen to be pulled into GGTT
mmaps. That is when we unbind the userptr object (on mmu invalidation),
we revoke all CPU mmaps, which may then recurse into mmu invalidation.

We looked for ways of breaking the cycle, but the revocation on
invalidation is required and cannot be avoided. The only solution we
could see was to not allow such GGTT bindings of userptr objects in the
first place. In practice, no one really wants to use a GGTT mmapping of
a CPU pointer...

Just before Daniel's explosive lockdep patches land in v5.4-rc1, we got
a genuine blip from CI:

On an system using ACPI hotplug the host router gets hotplugged first and then
the firmware starts sending notifications about connected devices so the above
scenario should not happen in reality. However, after taking a second
look at commit a03e828 ("thunderbolt: Serialize PCIe tunnel
creation with PCI rescan") that introduced the locking, I don't think it
is actually correct. It may have cured the symptom but probably the real
root cause was somewhere closer to PCI stack and possibly is already
fixed with recent kernels. I also tried to reproduce the original issue
with the commit reverted but could not.

So to keep lockdep happy and the code bit less complex drop calls to
pci_lock_rescan_remove()/pci_unlock_rescan_remove() in
tb_switch_set_authorized() effectively reverting a03e828.

Original version of g920_get_config() contained two kind of actions:

    1. Device specific communication to query/set some parameters
       which requires active communication channel with the device,
       or, put in other way, for the call to be sandwiched between
       hid_device_io_start() and hid_device_io_stop().

    2. Input subsystem specific FF controller initialization which, in
       order to access a valid 'struct hid_input' via
       'hid->inputs.next', requires claimed hidinput which means be
       executed after the call to hid_hw_start() with connect_mask
       containing HID_CONNECT_HIDINPUT.

Location of g920_get_config() can only fulfill requirements for #1 and

To solve this issue:

   1. Split g920_get_config() such that all of the device specific
      communication remains a part of the function and input subsystem
      initialization bits go to hidpp_ff_init()

   2. Move call to hidpp_ff_init() from being a part of
      g920_get_config() to be the last step of .probe(), right after a
      call to hid_hw_start() with connect_mask containing
      HID_CONNECT_HIDINPUT.

All bonding device has same lockdep key and subclass is initialized with
nest_level.
But actual nest_level value can be changed when a lower device is attached.
And at this moment, the subclass should be updated but it seems to be
unsafe.
So this patch makes bonding use dynamic lockdep key instead of the
subclass.

The WARN_ON_ONCE is introduced on truncate_folio_batch_exceptionals() to
capture whether the filesystem has removed all DAX entries or not.

And the fix has been applied on the filesystem xfs and ext4 by the commit
0e2f80a ("fs/dax: ensure all pages are idle prior to filesystem
unmount").

Apply the missed fix on filesystem fuse to fix the runtime warning:

Fix by converting cq_lock to a spinlock. All operations under the new

Reject migration of SEV{-ES} state if either the source or destination VM
is actively creating a vCPU, i.e. if kvm_vm_ioctl_create_vcpu() is in the
section between incrementing created_vcpus and online_vcpus.  The bulk of
vCPU creation runs _outside_ of kvm->lock to allow creating multiple vCPUs
in parallel, and so sev_info.es_active can get toggled from false=>true in
the destination VM after (or during) svm_vcpu_create(), resulting in an
SEV{-ES} VM effectively having a non-SEV{-ES} vCPU.

The issue manifests most visibly as a crash when trying to free a vCPU's
NULL VMSA page in an SEV-ES VM, but any number of things can go wrong.

Deliberately don't check for a NULL VMSA when freeing the vCPU, as crashing
the host is likely desirable due to the VMSA being consumed by hardware.
E.g. if KVM manages to allow VMRUN on the vCPU, hardware may read/write a
bogus VMSA page.  Accessing PFN 0 is "fine"-ish now that it's sequestered
away thanks to L1TF, but panicking in this scenario is preferable to
potentially running with corrupted state.

If the PHY driver uses another PHY internally (e.g. in case of eUSB2,
repeaters are represented as PHYs), then it would trigger the following
lockdep splat because all PHYs use a single static lockdep key and thus
lockdep can not identify whether there is a dependency or not and
reports a false positive.

Make PHY subsystem use dynamic lockdep keys, assigning each driver a
separate key. This way lockdep can correctly identify dependency graph
between mutexes.

If "try_verify_in_tasklet" is set for dm-verity, DM_BUFIO_CLIENT_NO_SLEEP
is enabled for dm-bufio. However, when bufio tries to evict buffers, there

Bug #1:
The zeroeth element of ctrl->pconfig[] is supposed to be unused.  We
start counting at 1.  However this code sets ctrl->pconfig[0].ch_mask = 128.

Bug #2:
There are SLIM_MAX_TX_PORTS (16) elements in tx_ch[] array but only
QCOM_SDW_MAX_PORTS + 1 (15) in the ctrl->pconfig[] array so it corrupts
memory like Yongqin Liu pointed out.

Bug 3:
Like Jie Gan pointed out, it erases all the tx information with the rx
information.

The commit under the Fixes tag added a netdev_assert_locked() in
bnxt_free_ntp_fltrs().  The lock should be held during normal run-time
but the assert will be triggered (see below) during bnxt_remove_one()
which should not need the lock.  The netdev is already unregistered by
then.  Fix it by calling netdev_assert_locked_or_invisible() which will
not assert if the netdev is unregistered.

When a user creates a dualpi2 qdisc it automatically sets a timer. This
timer will run constantly and update the qdisc's probability field.
The issue is that the timer acquires the qdisc root lock and runs in
hardirq. The qdisc root lock is also acquired in dev.c whenever a packet
arrives for this qdisc. Since the dualpi2 timer callback runs in hardirq,
it may interrupt the packet processing running in softirq. If that happens
and it runs on the same CPU, it will acquire the same lock and cause a
deadlock. The following splat shows up when running a kernel compiled with

Commit 3c7ac40 ("scsi: ufs: core: Delegate the interrupt service
routine to a threaded IRQ handler") introduced an IRQ lock inversion
issue. Fix this lock inversion by changing the spin_lock_irq() calls into
spin_lock_irqsave() calls in code that can be called either from
interrupt context or from thread context. This patch fixes the following

If preparing a write bio fails then blk_zone_wplug_bio_work() calls
bio_endio() with zwplug->lock held. If a device mapper driver is stacked
on top of the zoned block device then this results in nested locking of
zwplug->lock. The resulting lockdep complaint is a false positive
because this is nested locking and not recursive locking. Suppress this
false positive by calling blk_zone_wplug_bio_io_error() without holding
zwplug->lock. This is safe because no code in
blk_zone_wplug_bio_io_error() depends on zwplug->lock being held. This
patch suppresses the following lockdep complaint:

In gicv5_irs_of_init_affinity() a WARN_ON() is triggered if:

 1) a phandle in the "cpus" property does not correspond to a valid OF
    node
 2  a CPU logical id does not exist for a given OF cpu_node

#1 is a firmware bug and should be reported as such but does not warrant a
   WARN_ON() backtrace.

#2 is not necessarily an error condition (eg a kernel can be booted with
   nr_cpus=X limiting the number of cores artificially) and therefore there
   is no reason to clutter the kernel log with WARN_ON() output when the
   condition is hit.

Rework the IRS affinity parsing code to remove undue WARN_ON()s thus
making it less noisy.

When the "proxy" option is enabled on a VXLAN device, the device will
suppress ARP requests and IPv6 Neighbor Solicitation messages if it is
able to reply on behalf of the remote host. That is, if a matching and
valid neighbor entry is configured on the VXLAN device whose MAC address
is not behind the "any" remote (0.0.0.0 / ::).

The code currently assumes that the FDB entry for the neighbor's MAC
address points to a valid remote destination, but this is incorrect if
the entry is associated with an FDB nexthop group. This can result in a
NPD [1][3] which can be reproduced using [2][4].

Fix by checking that the remote destination exists before dereferencing
it.

Problem description
===================

Lockdep reports a possible circular locking dependency (AB/BA) between
&pl->state_mutex and &phy->lock, as follows.

phylink_resolve() // acquires &pl->state_mutex
-> phylink_major_config()
   -> phy_config_inband() // acquires &pl->phydev->lock

whereas all the other call sites where &pl->state_mutex and
&pl->phydev->lock have the locking scheme reversed. Everywhere else,
&pl->phydev->lock is acquired at the top level, and &pl->state_mutex at
the lower level. A clear example is phylink_bringup_phy().

The outlier is the newly introduced phy_config_inband() and the existing
lock order is the correct one. To understand why it cannot be the other
way around, it is sufficient to consider phylink_phy_change(), phylink's
callback from the PHY device's phy->phy_link_change() virtual method,
invoked by the PHY state machine.

phy_link_up() and phy_link_down(), the (indirect) callers of
phylink_phy_change(), are called with &phydev->lock acquired.
Then phylink_phy_change() acquires its own &pl->state_mutex, to
serialize changes made to its pl->phy_state and pl->link_config.
So all other instances of &pl->state_mutex and &phydev->lock must be
consistent with this order.

Problem impact
==============

I think the kernel runs a serious deadlock risk if an existing
phylink_resolve() thread, which results in a phy_config_inband() call,
is concurrent with a phy_link_up() or phy_link_down() call, which will
deadlock on &pl->state_mutex in phylink_phy_change(). Practically
speaking, the impact may be limited by the slow speed of the medium
auto-negotiation protocol, which makes it unlikely for the current state
to still be unresolved when a new one is detected, but I think the
problem is there. Nonetheless, the problem was discovered using lockdep.

Proposed solution
=================

Practically speaking, the phy_config_inband() requirement of having
phydev->lock acquired must transfer to the caller (phylink is the only
caller). There, it must bubble up until immediately before
&pl->state_mutex is acquired, for the cases where that takes place.

phylink_major_config() caller #1, phylink_mac_initial_config(), does not
acquire &pl->state_mutex nor do its callers. It must acquire
&pl->phydev->lock prior to calling phylink_major_config().

phylink_major_config() caller #2, phylink_resolve() acquires
&pl->state_mutex, thus also needs to acquire &pl->phydev->lock.

phylink_major_config() caller #3, phylink_ethtool_ksettings_set(), is
completely uninteresting, because it only calls phylink_major_config()
if pl->phydev is NULL (otherwise it calls phy_ethtool_ksettings_set()).
We need to change nothing there.

Other solutions
===============

The lock inversion between &pl->state_mutex and &pl->phydev->lock has
occurred at least once before, as seen in commit c718af2 ("net:
phylink: fix ethtool -A with attached PHYs"). The solution there was to
simply not call phy_set_asym_pause() under the &pl->state_mutex. That
cannot be extended to our case though, where the phy_config_inband()
call is much deeper inside the &pl->state_mutex section.

5da3d94 ("PCI: mvebu: Use for_each_of_range() iterator for parsing
"ranges"") simplified code by using the for_each_of_range() iterator, but
it broke PCI enumeration on Turris Omnia (and probably other mvebu
targets).

Issue #1:

To determine range.flags, of_pci_range_parser_one() uses bus->get_flags(),
which resolves to of_bus_pci_get_flags(), which already returns an
IORESOURCE bit field, and NOT the original flags from the "ranges"
resource.

Then mvebu_get_tgt_attr() attempts the very same conversion again.  Remove
the misinterpretation of range.flags in mvebu_get_tgt_attr(), to restore
the intended behavior.

Issue #2:

The driver needs target and attributes, which are encoded in the raw
address values of the "/soc/pcie/ranges" resource. According to
of_pci_range_parser_one(), the raw values are stored in range.bus_addr and
range.parent_bus_addr, respectively. range.cpu_addr is a translated version
of range.parent_bus_addr, and not relevant here.

Use the correct range structure member, to extract target and attributes.
This restores the intended behavior.

This is of course no good, but is at odds with how LPI refcounts are
managed. Solve the locking mess by deferring the release of unreferenced
LPIs after the ap_list_lock is released. Mark these to-be-released LPIs
specially to avoid racing with vgic_put_irq() and causing a double-free.

Since references can only be taken on LPIs with a nonzero refcount,
extending the lifetime of freed LPIs is still safe.

When you run a KVM guest with vhost-net and migrate that guest to
another host, and you immediately enable postcopy after starting the
migration, there is a big chance that the network connection of the
guest won't work anymore on the destination side after the migration.

The "FAULT_FLAG_ALLOW_RETRY missing" indicates that handle_userfaultfd()
saw a page fault request without ALLOW_RETRY flag set, hence userfaultfd
cannot remotely resolve it (because the caller was asking for an immediate
resolution, aka, FAULT_FLAG_NOWAIT, while remote faults can take time).
With that, get_map_page() failed and the irq was lost.

We should not be strictly in an atomic environment here and the worker
should be sleepable (the call is done during an ioctl from userspace),
so we can allow adapter_indicators_set() to just sleep waiting for the
remote fault instead.

This attemps to fix possible UAFs caused by struct mgmt_pending being
freed while still being processed like in the following trace, in order
to fix mgmt_pending_valid is introduce and use to check if the
mgmt_pending hasn't been removed from the pending list, on the complete
callbacks it is used to check and in addtion remove the cmd from the list
while holding mgmt_pending_lock to avoid TOCTOU problems since if the cmd
is left on the list it can still be accessed and freed.

Since blamed commit, unregister_netdevice_many_notify() takes the netdev
mutex if the device needs it.

If the device list is too long, this will lock more device mutexes than
lockdep can handle:

Add a helper to close and then unlock a list of net_devices.
Devices that are not up have to be skipped - netif_close_many always
removes them from the list without any other actions taken, so they'd
remain in locked state.

Close devices whenever we've used up half of the tracking slots or we
processed entire list without hitting the limit.

Expand the prefault memory selftest to add a regression test for a KVM bug
where KVM's retry logic would result in (breakable) deadlock due to the
memslot deletion waiting on prefaulting to release SRCU, and prefaulting
waiting on the memslot to fully disappear (KVM uses a two-step process to
delete memslots, and KVM x86 retries page faults if a to-be-deleted, a.k.a.
INVALID, memslot is encountered).

To exercise concurrent memslot remove, spawn a second thread to initiate
memslot removal at roughly the same time as prefaulting.  Test memslot
removal for all testcases, i.e. don't limit concurrent removal to only the
success case.  There are essentially three prefault scenarios (so far)
that are of interest:

 1. Success
 2. ENOENT due to no memslot
 3. EAGAIN due to INVALID memslot

For all intents and purposes, #1 and #2 are mutually exclusive, or rather,
easier to test via separate testcases since writing to non-existent memory
is trivial.  But for #3, making it mutually exclusive with #1 _or_ #2 is
actually more complex than testing memslot removal for all scenarios.  The
only requirement to let memslot removal coexist with other scenarios is a
way to guarantee a stable result, e.g. that the "no memslot" test observes
ENOENT, not EAGAIN, for the final checks.

So, rather than make memslot removal mutually exclusive with the ENOENT
scenario, simply restore the memslot and retry prefaulting.  For the "no
memslot" case, KVM_PRE_FAULT_MEMORY should be idempotent, i.e. should
always fail with ENOENT regardless of how many times userspace attempts
prefaulting.

Pass in both the base GPA and the offset (instead of the "full" GPA) so
that the worker can recreate the memslot.
