fix(idempotency): update v8 coverage ignore hint for vitest 4.1.0

The new ast-v8-to-istanbul used by vitest 4.1.0 no longer supports
inline `v8 ignore next` for arrow function bodies. Switch to
`v8 ignore start/stop` block syntax instead.

Fixed the coverage failure in packages/idempotency/src/middleware/makeHandlerIdempotent.ts (line 125).

Vitest 4.1.0 replaced v8-to-istanbul with ast-v8-to-istanbul for coverage reporting. The old library used regex-based text matching for ignore hints, while the new one does AST-level analysis. The inline /* v8 ignore next -- @preserve */ pragma was no longer being respected for the dummy arrow function () => ({}).

The fix switches to the /* v8 ignore start */ / /* v8 ignore stop */ block syntax, which is properly supported by the new AST-based coverage tool (added in vitest-dev/vitest#9204).
