    Triaged report 6EwKzKYIitVuDYUV3kqKa (CLI 0.8.2, OpenAI gpt-5.4). This is an outbound tool-history integrity failure. Current main already has recovery in ReactLoopRunner from commit 87dc29a, plus context-crop coverage, but that does not establish that this reported history is repaired. The report omits the outbound message sequence needed to reproduce the missing results.

    Keeping this issue open. Next step: reproduce the reported assistant/tool sequence, including context cropping or session resume, and assert that every tool call has its result in the actual OpenAI request. The feedback/reporting PR does not change this provider path. The identical auto-report #629 is being consolidated here.
