This PR adds an "edit this page on GitHub" link, and a "report issue" link to the sidebar of every page to quickly get to the edit or new issue view for the page in the GitHub interface. It does so by customizing the "minimal" theme's default layout, which was copied into the codebase from the latest upstream commit at the time of writing.
The commits in this PR are meant to be preserved.
