API Error: 500 Internal server error on every new request

What's Wrong?

Every new request fails with 500 Internal Server Error.
Issue is reproducible on clean fresh requests, not related
to context size or conversation history.

Request should return a valid response from the API.

Steps to Reproduce

1. Open Claude Code
2. Start a new session / fresh chat
3. Send any message (e.g. "hello")
4. Immediately get 500 Internal Server Error
5. Reproducible on every attempt, multiple times

Additional Information

Error persists across multiple sessions and fresh restarts.
Issue started today April 15, 2026. API status page shows
operational but error is consistent.
