[Bug] Temporary directory access permissions not enforced

Bug Description
"Yes, and always allow access to [any folder]/ from this project" does absolutely nothing. It is extremely flaky. I've added it to additionalDirectories in settings.json, I've added it to allowed-tools: in the skill that uses it, I've tried everything. There is a nasty bug here and it's infuriating 😡.
