P2 Badge Replace placeholder evidence paths with actual references

The resolution states that parity evidence was documented, but the Evidence Paths entries are still placeholder \ values (and Upstream ref is similarly empty), so a reviewer cannot trace this issue to concrete source artifacts. In the lane-tracking workflow, this makes #421 look resolved without any verifiable linkage, which undermines auditability and follow-up execution.
