Improve layout of title column in Extension Manager

Currently, the left column in Extension Manager looks kind of messy to me, because the line with all the secondary info (version number, author / link, and date) is typically really long (and wraps) compared to the title (see below).

I think we should consider:

• Putting the version number immediately after the title, but in a smaller font (like on the main registry)
• If there's an author field, don't show the github userid--just link the author name to the github profile

Not sure why the dates aren't showing up in the Installed Extensions tab though.

FYI, the publish dates don't show up in Installed Extensions since we don't currently copy that bit of metadata from the server.
