Adds an 'Excluded' column (Yes/No) + an 'Excluded PRs (not counted in coverage)' summary to the CSV, and an [EXCLUDED] marker on each PR block in the PDF, so the export is a full audit record that clearly distinguishes counted vs excluded PRs. The excluded-flag lookup is guarded so the export works standalone.
