Describe the bug
Remake of CCNA #8061

• Currently, the is no way to know if the instrument is missing DE or DDE from the UI. You need to download the CSV and filter by comment ID, which is not user friendly.

Expected behaviour
A new column should be added to specify if it is DE or DDE missing.
Can be called "Data entry type"

Usually initial data entry is done first, but in practice it does not really matter. What is important is that the initial and double data entry is done by 2 different individuals.
I could see cases where the double data entry staffs are more efficient than the coordinator at a site and so the DDE is done before the initial data entry... Does that answer your question?
