[survey_accounts] Error 500 when trying to complete 'Mild Behavioral Impairment Checklist (MBI-C) (CTU)'

Describe the bug
Error 500 is given when clicking 'Save and Continue' on first page of the 'Mild Behavioral Impairment Checklist (MBI-C) (CTU)' survey. I tried various candidates and entries to the fields and all give error 500.

To Reproduce
Steps to reproduce the behavior (attach screenshots if applicable):

1. Go to the survey accounts module
2. Search for 'Mild Behavioral Impairment Checklist (MBI-C) (CTU)'
3. Click on URL for 'Mild Behavioral Impairment Checklist (MBI-C) (CTU)'
4. Fill out fields on first page
5. Click the 'Save and Continue' button
