Fix `action_info_hash_key_to_awaited_action out of sync` Error

Users have reported that when using the dedup store with a disk index that this error occurs when performing an upgrade.

We need to investigate and look into what's causing this error.

I don't believe this issue occurs exclusively post-upgrade. At least, I have purged the on-disk state of my container post-upgrade and I still see this error in my logs.

Update: I haven't seen this error in my logs for at least a month. It's possible it no longer occurs for me.
