Uploading a new primary file can fail but move the current one to archived but not set the upload as PRIMARY nor does and action_log entry show

This is sporadic and I have not been able to replicate.
A retry of the upload usually succeeds but this can cause frustration for students in end of semester panic.
This is pure speculation but it might happen more under heavy load.
