Header is not visible!

The header is not visible!
I gave all the needed permission
It is not viable for all profiles including admin

You have to select it in the Header tab of survey and make sure it's on

What do you mean by selecting the header tab? we have added a header. we made all the settings but still, it does not display. What steps should we take exactly?

Then someone need to Apex debug it and see why it's not selecting header. The code to show survey is at https://github.com/SalesforceLabs/survey-force/blob/main/force-app/main/default/pages/TakeSurvey.page

It's controlled by Hide_Survey_Name__c custom field on the survey. Make sure that field is NOT checked in the survey "details"
