refactor: Refactor authentication and permission middleware for error handling and security

I plan to do the following changes:
I re-factored the code for authentication and permission middleware for more effective error handling.
Improved logging for easier debugging and monitoring in production.
Some more effort was put in behind securing the request headers.
The structure was improved to allow for better maintainability.
