Add approved by user and approval date to extension requests

Issue Description

When a super_user approves a extension request it just shows a approved button. It needs to show name of the approver and date of approval.

Expected Behavior

When a super_user approves an extension request:

1.The name of the approver (super_user) should be visible.
2.The number of days of approval should be displayed alongside the approver name and hovering over the days should display the exact timestamp of approval.

Current Behavior

Currently, when a super_user approves an extension request, only an 'approved' button is shown. There is no information about who approved the extension or when it was approved.
