Fix auto-format dataset for message_type=completions

Description

Previously, envs that use message_type=completions would fail because the required field example_id is not set up correctly.

uv pip install -e environments/continuation_quality/

uv run vf-eval continuation_quality -n1 -r1 -v

This PR formats completions datasets by setting up the example id and task are correctly set.

Testing

• All existing tests pass when running uv run pytest locally.
