Design deployment process

Design the way we are gonna do deployment.

Blocked by #169.
Some thoughts:

• Different environments: dev (probably no deployment), test (do we need it?), prod
• Think about having multiple therapist accounts for dev, test, prod, we need to request it from niceday.
