This shouldn't have any functional changes, it's just a refactor of the ID generation code to allow for creating an ID from individual parts (time, sequence, machine_id, pid).

for a project that I'm migrating from MongoDB to RethinkDB, and I wanted to be able to map the IDs one-to-one to easily check if a document had been migrated. I added the other methods just for completeness)
