[NDR-333] Update FHIR lambdas to use the CORE dynamo db

Description

Updates the FHIR endpoints and virus Document Reference to use the new core table. Also other changes around how tables are determined.

Context

5 GSIs were used previously for items we would never need to find. Also streamlining for search to use a pk/sk set up.
