[Bug]: Permissions: When adding Popular network or a custom network via chainlist.org, the 'Network RPC URL Review' modal is triggered, displaying two RPC URLs

Describe the bug

When adding a custom network via chainlist.org, the 'Network RPC URL Review' modal is triggered, displaying two RPC URLs. One of these URLs appears to be unrelated to the selected network, consistently showing as https://flashbots.polygon-mainnet.com/ for any new network addition.

The Blast chain icon and the 'Review' button have padding issues.

The network switch toast notification does not display the chain icon.
