Update groupId and package names from com.ibm.fhir to org.linuxforhealth.fhir

Is your feature request related to a problem? Please describe.
We've already moved the code and some high-level branding. We should change the package name as well prior to our 5.0 so that we don't need to cut a 6.0 when they change in the coming months.

Update the package names and the docker namespace too.
