[Bug]: Tool call fails after latest update (Bedrock) #10428

Description

@dvejsada

What happened?

I have updated LibreChat to the latest dev and every tool call fails with the following logs:

2025-11-10 09:35:09 error: [api/server/controllers/agents/client.js #sendCompletion] Operation aborted No step ID found
2025-11-10 09:35:09 error: [api/server/controllers/agents/client.js #sendCompletion] Unhandled error type No step ID found
Error in handler EventStreamCallbackHandler, handleChainEnd: Error [ERR_INTERNAL_ASSERTION]: This is caused by either a bug in Node.js or incorrect usage of Node.js internals.
Please open an issue with this stack trace at https://github.com/nodejs/node/issues

Only invocations that do not require tool call goes throught.

After downgrade to latest rc released 10 days back everything works fine, therefore definitely an issue with latest-dev.

I just did a fresh install on a digital ocean droplet with the latest release and I'm experiencing issues as well.

Nginx reports a bad gateway and cpu usage of the droplet spikes to 100%

Unfortunately I can’t reproduce this so more information about your setup would help. Are you using a custom endpoint? Any specific “router”?

I think this is unrelated and you should open your own post with as many details as possible.

We are using agents with AWS Bedrock models (primarily latest Anthropic Haiku and Sonnet). The issues started right after the update to the latest-dev. When I rolled back to the latest rc, the issues disappeared, therefore it is definitely not a provider or tool issue.

What other information would be helpful? I have not found anything else in the logs then pasted above.

I have also tried on my personal deployment with direct Anthropic and OpenAI based agents. The Anthropic based agents work fine (use tools, no errors), however the OpenAI based agents (tried both GPT-5 and GPT-5-mini) ignore any connected tools (stating they have no tools to call despite having the same MCP servers connected as used by Anthropic agents moments before and after, tried removing and re-attaching tools, nothing worked). So there is definitely something wrong.

This is more information than originally reported, thanks for providing that.

    Not able to reproduce:

I'm looking into it based off the additional information and the original error message, thanks for the report.

I believe I'm seeing the same issue - just updated my local LibreChat install today. It actually kills the LibreChat application and kicks me back to the command line.
