Fix PHP warning trying to access array offset on true in settings.php

• Removed redundant require_once for config.php since header.php already provides it. The redundant call resulted in the boolean 'true' overwriting the $config array, leading to the array offset warning.
• Added the null-coalescing operator '??' to htmlspecialchars calls to prevent the 'Passing null to parameter 🛡️ Sentinel: [CRITICAL] Fix command injection #1 ($string) of type string is deprecated' notice that occurred when variables were legitimately missing.
